🏥 Registration & Arrival Information

Hospital: Chi Mei Medical Center (Main Campus in Yongkang)


Location: Examination Room, 2nd Floor, Second Medical Building.


Check-in: Please arrive 30 minutes before your scheduled procedure. Bringing a family member for accompaniment is highly recommended.


Parking: On-site parking is very limited. No parking spaces are usually available after 09:00 AM. We strongly recommend taking a taxi or public transportation.

📋 Pre-Operative Preparation

Vocal Cord Assessment: A vocal cord examination will be performed before the procedure.


Diet on the Day of Surgery:

Morning Session: Eat breakfast as usual. Take your prescribed oral medication after the meal.


Afternoon Session: Eat breakfast and lunch as usual. Take your prescribed oral medication after lunch.


(Note: Oral medications refer to the anti-inflammatories, anti-swelling, and painkillers prescribed during your clinic visit.)


⚠️ Medication Suspension (Crucial): If you are taking blood thinners (Anticoagulants/Antiplatelets, including TCM or Western medicine), health supplements, or Fish Oil (Omega-3), you must strictly stop taking them 2 weeks prior to the surgery.


No Smoking: Do NOT smoke (including e-cigarettes) on the day of surgery. Your cooperation is essential.


Pacemakers: If you have a cardiac pacemaker, inform your doctor in advance. The ablation needle used for patients with pacemakers is different and requires special arrangements.
💄 Personal Care & Requirements

Nails: Please remove nail polish and gel nails at least one day before (required for monitoring blood oxygen levels).


Appearance: Do not wear makeup or jewelry (necklaces, rings, earrings, etc.).


Clothing: You will change into a surgical gown before the procedure. You may bring a personal bag to store your clothes.


Documents: Bring your ID and payment receipt. These will be returned after the staff verifies your identity.

✨ Well-being & Follow-up

Rest: Maintain a relaxed mood. Get plenty of sleep and rest during the week before the procedure. Being in good physical condition reduces pain sensitivity and aids recovery.


Confirmation: We will call you 1 to 14 days before the surgery to confirm the time. If we cannot reach you, we will send an SMS.


Punctuality: The examination room is scheduled for other departments afterward. Delays may affect the success of your procedure schedule.

Esta guía es para pacientes que se someterán al procedimiento de ablación térmica de nódulos tiroideos en nuestra institución.

🏥 Información de Registro
Lugar: Segundo piso, Segundo Edificio Médico (Second Medical Building), Sala de exámenes.

Hora: Favor de presentarse 30 minutos antes de la cirugía. Se recomienda venir acompañado.

Transporte: El estacionamiento del hospital es limitado. Después de las 09:00 a.m. suele estar lleno. Se sugiere tomar taxi o transporte público.

📋 Instrucciones Previas
Evaluación de cuerdas vocales: Se realizará una revisión previa del estado de su voz.
Alimentación el día de la cirugía:
Turno Mañana: Desayune normalmente y tome su medicamento después de comer.
Turno Tarde: Desayune y almuerce normalmente; tome su medicamento después del almuerzo.
(Nota: Se refiere a analgésicos o antiinflamatorios recetados en consulta).

⚠️ Medicamentos Especiales: Si toma anticoagulantes, medicina tradicional china, suplementos nutricionales o aceite de pescado, debe suspenderlos 2 semanas antes del procedimiento.

Prohibido Fumar: No fume (incluye cigarrillos electrónicos) el día de la cirugía.

Marcapasos: Si usa marcapasos, avísenos de inmediato; requerimos un equipo especial para su seguridad.

Preparación Personal:

Retire el esmalte de uñas o gelish un día antes (para monitoreo de oxígeno).

No use maquillaje, perfumes ni joyas (anillos, collares).

Se le entregará una bata quirúrgica; puede traer una bolsa para su ropa.

3.【嚴格禁菸】手術當天請勿抽菸（含電子菸），請務必配合。

4.若您有裝置心臟節律器，請務必事先告知並與醫師討論，因所使用的消融針將與一般人不同，需特別安排。

5.請於至少前一日卸除指甲油及光療指甲（以便監測血氧）。請勿化妝及配戴任何飾品（項鍊、戒指、耳環等）。術前會更換手術服，可自備一個提袋收納個人衣物。

6. 攜帶文件：攜帶證件與繳費收據，報到時檢查室人員核對身份後歸還。

7. 請維持輕鬆愉快的心情，術前一週內記得多休息、保持充足睡眠。當身體狀態良好時，術中的感受會更輕鬆（痛感較少），對術後的恢復也很有幫助！

8. 聯繫確認：手術前 1～14 天內，我們將以電話提醒手術時間；如未接通，會改以簡訊通知。

9. 準時報到：因檢查室後續時段將安排其他科別使用，逾時可能影響手術進行。

甲狀腺問與答(第三集) 甲狀腺結節可以吃海帶嗎？

蠻多病人一聽到自己有甲狀腺結節，就開始完全禁止自己吃任何的昆布海帶類，即使醫生已經跟他說可以吃，他還是堅信絕對不能吃，所以在這邊要跟大家做個說明。 也有很多人說鄰居說不能吃海帶，比較相信鄰居不相信醫生。

為什麼會有大脖子？ 缺碘的地區會造成甲狀腺腫，也就是俗稱的「大脖子」。如果碘不足，我們身體就會想辦法讓甲狀腺組織增生腫大，這樣才可以製造出足夠的甲狀腺素，以供應身體的代謝使用。

台灣的食鹽政策 政府在很久以前就已經全面實施「食鹽加碘」。自從有了這項政策，以往甲狀腺腫的盛行率降低非常多。實際上，現在台灣幾乎看不到因為缺碘而造成的大脖子，因為我們目前屬於飲食中「碘充足」區域。 除非你長期給自己低碘飲食。

結節病人的飲食建議

1. 甲狀腺功能正常： 如果是單純有結節但功能正常的病人，是可以吃海帶昆布類的喔！食用鹽的部分，也建議使用「有碘鹽」。甲狀腺功能低下的人，也適用正常飲食。

2. 甲狀腺機能亢進： 這類病人才需要避開含碘較高的食物，並且改用「無碘鹽」。

注意市售進口鹽 現在市面上有很多國外進口的鹽（如玫瑰鹽、岩鹽），很多是不含碘的。購買時請注意成分標示，請記得「碘」也是我們飲食中必須的養分。

請不要一檢查出甲狀腺結節，就自己堅持要吃低碘飲食或完全禁止吃海帶昆布。功能正常的甲狀腺結節病人，適量攝取碘才是維持甲狀腺健康的做法。

很多人聽過「甲狀腺」，但你知道它在身體裡扮演什麼角色，又長什麼樣子嗎？今天帶大家快速認識這個重要器官！

它的外型與位置

甲狀腺位於我們脖子前方，長得就像一隻漂亮的蝴蝶。左右兩翼： 稱為左葉與右葉，由中間一塊「峽部」相連。
體型嬌小： 正常情況下只有 15 到 20 公克（大約像兩顆大草莓那麼重）。
潛力驚人： 如果甲狀腺生病了（俗稱大脖子），它甚至可以腫大到好幾百公克！

超高流量的「血液中心」

甲狀腺是全身血液循環最豐富的器官之一，它每公克的血流量甚至比腎臟還多！ 這就是為什麼當甲狀腺機能亢進（如葛瑞夫茲氏症）時，因為血流極強，醫師甚至可以用聽診器在脖子聽到血液流動的聲音。

放大鏡下的秘密：無數的小倉庫

如果把甲狀腺放大來看，它是由無數個圓形小球組成的，這叫做「濾泡」。
工廠與倉庫： 這些濾泡中心充滿了膠狀物質，可以把它想像成一個個儲存荷爾蒙原料的倉庫。
自動化生產： 當身體需要能量時，濾泡細胞就會動起來，從倉庫搬出原料，加工成甲狀腺素送往全身。

它是如何工作的？

在精密電子顯微鏡下，這些細胞非常忙碌：靈敏觸手： 細胞表面有許多細小微絨毛，像觸手一樣負責抓取原料、分泌成品。
接收指令： 當大腦發出指令（TSH 訊號），細胞就會開始擴張、加班，把倉庫裡的物資轉換成體內的能量。

最近很紅的「瘦瘦針」，跟甲狀腺癌有關嗎？

除了製造代謝荷爾蒙的濾泡細胞，甲狀腺裡還躲著一群神祕配角，叫做 「C 細胞（Parafollicular C cells）」：不同的功能： 它們負責製造「降鈣素」，調節體內鈣質平衡。
獨特的長相： 它們不住在倉庫（濾泡）裡，而是待在細胞間的縫隙，且體力特別好（含有大量線粒體）。
健康警戒線： 如果 C 細胞異常增生，可能演變成「甲狀腺髓質癌（MTC）」。

關於瘦瘦針的評估： 目前很紅的「瘦瘦針」（GLP-1 受體促效劑），仿單上的確有標註關於 MTC 的風險警告，這就與 C 細胞息息相關。特定的風險： 研究發現藥物可能影響 C 細胞，在動物身上誘發「甲狀腺髓質癌」。不過，目前證據顯示在人類身上可能還沒有觀察到這樣的現象（未來呢？我不知道）。

誰要特別注意？
若您或家人曾患有「甲狀腺髓質癌」，或帶有「多發性內分泌腫瘤症候群（MEN 2）」遺傳基因，就不建議使用。
對一般人來說這種癌症不算常見，不建議過度恐慌。

甲狀腺微創消融手術前會執行病理切片嗎？

在台灣，甲狀腺微創消融手術前的標準評估方式，絕大多數是執行「細針抽吸細胞學」(Fine-Needle Aspiration Cytology, FNAC) 檢查。

這和「病理切片」(Biopsy) 是不同的。臨床上所指的「病理切片」常對應到「粗針組織採樣」(Core Needle Biopsy)，這種檢查使用的針具較粗，侵入性及傷害相對較高，根據醫學上「不傷害原則」(Do no harm)，「病理切片」通常不是甲狀腺結節微創消融的常規第一線檢查。

問題：請問甲狀腺細針抽吸的時候可以呼吸嗎？

回答：

可以的！請放輕鬆，保持正常呼吸。

細針抽吸請全程保持平穩、正常的呼吸，比平常呼吸稍微小力一點。

醫師下針之後請不要吞嚥（吞口水）、不要講話、不要突然咳嗽。



為什麼不能吞嚥或講話？
這是因為我們的甲狀腺會緊貼著氣管和食道，當我們做「吞嚥」或「講話」這些動作時，喉嚨會帶動甲狀腺跟著上下移動。

所以，檢查的流程通常是：
1. 先用超音波把整個甲狀腺都看過。
2. 超音波導引下將針插入甲狀腺結節內。
3. 下針後，病人暫時不要吞嚥、不要講話，但維持正常呼吸。
4. 抽吸完成，醫師就會跟您說針拿出來了，這時候可以吞口水、也可以講話了。

您唯一要做的就是「放鬆、正常呼吸」，不要因為緊張而扭來扭去或是閃躲。
